Provider Search
Welcome to the Provider Search Tool. Provider Search is a listing of all the providers certified to provide services to people with DD in Clark County. This database is a shortened version of the one our staff uses when searching for providers. Your SSA has access to the full version which has more columns showing available services from each provider. Once you have found a provider, contact your SSA to discuss service authorization.

How to Search
- You can adjust the number of entries you can see on a page, and scroll through them up and down, and left and right.
- If you are looking for a specific name, type of provider, or waiver, you can use the Search box to enter that for which you are looking (eg. IO, agency, or Joe) Capitalization doesn’t matter. Spelling does.
- Use the key provided to know for what the column abbreviations stand.
- You can sort by column by clicking the small arrow next to each column heading. For example, if you want to see all the Adult Day providers, you can click the tiny arrow to the right of “ADS” so that it is pointing down and all the ADS providers will be sorted to the top of the list.
