Information and Resources for Providers

Provider certification is managed by the Ohio Department of Developmental Disabilities (DODD).  Visit their website to learn about becoming a provider.

You will need to get a background check in order to become a provider. Clark DD offers these for staff and community providers in our HR office. You can contact us at 937-346-0747 to schedule a background check.

Prior to applying to DODD for certification, you must have 8 hours of initial training, including 2 hours of required DODD curriculum, plus CPR/FA. You can take the free initial 8-hour provider training online at DODD’s MyLearning or visit our training calendar for in-person training using DODD’s curriculum. If applying to provide Homemaker Personal Care, Participant-Directed Personal Care, or Nursing Services, you are also required to take the Electronic Visit Verification Training found on Ohio Medicaid’s website.
