Service and Support Administrator (SSA)

The Service and Support Administrator (SSA) is here to be your trusted partner and single point of contact, helping you navigate services and supports that fit your unique needs. Whether you live at home with your family, in your own home, a nursing facility, or a licensed group home, the SSA is committed to working alongside you to make sure you have the resources and assistance you need.

The SSA collaborates with other agencies and individuals when needed—such as partnering with your school district if your child is eligible and attends school—or may serve as the lead coordinator to bring all services together seamlessly. Our goal is to make the process easier for you and ensure your voice is heard every step of the way.

Assessment of Needs

The SSA partners with you to explore your unique needs and goals. Together, we review your situation, listen to your perspective, and include input from those who know you best. We also consider professional assessments and seek additional insights when helpful. Our purpose is to assist you in identifying supports that empower you to live your best life.

Facilitate Plan Development

Your voice leads the way. The SSA listens to you, your family, and your support network to understand what matters most and what possibilities exist for your future. We assist in building a strong team that believes in your potential and works creatively toward your goals. The SSA helps the team think "outside the box," encourages positive commitments, and facilitates collaboration. Together, we create an Individual Service Plan (OISP) that reflects your dreams, and the SSA authorizes services provided by the County Board to support your success.

Coordinate Services

The SSA assists you by simplifying complex systems and ensuring everyone understands your needs and goals. We help you and your team navigate professional terminology and make sure providers understand what matters most to you. The SSA links you to community resources, helps set priorities, and coordinates services so everything works together smoothly. We document these efforts to keep your plan on track.

Monitor Implementation

The SSA maintains an ongoing relationship with you and your team to ensure your plan supports your health, safety, and well-being. We follow up on commitments, monitor provider efforts, and assist in making adjustments when needed. If challenges arise, we work together to find solutions. The SSA also collaborates with Quality Assurance teams to address trends and ensure your supports meet the highest standards.

Waiver Administration

The SSA assists you in maintaining eligibility for waiver programs by managing official files, completing assessments, submitting financial materials, and authorizing payments promptly. We also help with waiting lists and support you in developing an individual budget that aligns with your goals. Our focus is on making these processes smooth and stress-free.

Crisis Intervention

Life can bring unexpected challenges, and we're here for you 24/7. If you or your family experience a crisis, the SSA on-call responds quickly and uses every available resource to resolve the situation. You can reach us through the Community Living Services emergency number or the community-wide crisis line. Our priority is your safety and well-being.

Advocacy

The SSA encourages you to know and exercise your rights, resolve conflicts, and use available resources to advocate for yourself. We assist you and your family in strengthening services, identifying gaps, and working together to improve supports. Your voice helps shape the future, and we're here to make sure it's heard.
